Why this role would be great for you
As a YMCA Together Support Practitioner you will be based in our Dispersed service. Your role will be to provide intensive support to those in the service who have experienced homelessness. You will have access to your own car due to the dispersed nature of the role. You will complete a through assessments of a client’s needs and be creative on how you support them to address those. You will also be able to expand on existing housing referral routes and empower your case load to look at alternative options to meet their permanent housing needs. You will understand the contributing factors that will have led to referrals into the service and use your previous working experiences to facilitate a good ending to a person’s homeless journey. You will have a genuine passion in getting to know people and building up positive relationships with those that you support, colleagues and partners throughout the city.
What your week may look like
- Providing support in people’s homes to identify and address any needs.
- To actively liaise with, housing services, other professionals and any other agencies that are working alongside those that you support.
- To take part in weekly team meetings, handovers and training as directed by your line manager.
- To be responsible for housing management tasks attached to the properties you will be working in.
- To be proactive in sourcing a move on pathway that is appropriate and meets the needs of the individuals you support – this could mean arranging meetings with third parties/housing providers to build up good professional relationships.
- At times, to support your colleagues with their caseloads if needed.

The skills we are looking for
- EXPERIENCE– Ideally you will have a background of working within a charity, community project and supporting people on a rehousing pathway.
- You will have knowledge around supporting people with Mental ill health, challenging and complex behaviours that could include including substance misuse and offending behaviours within a community setting.
- KNOWLEDGE – A good understanding of relevant and appropriate legislation, including a working knowledge of Housing Law, The care act and welfare benefits . An understanding of health and safety requirements in the workplace.
- COMMUNICATION – Excellent communication, fostering a respectful, collaborative and professional approach to the people who you work alongside always ensuring confidentiality.
- EMPATHY and RESPECT – The ability to empower people to make positive changes, showing empathy whilst maintaining professional boundaries
- COMMITMENT – The ability to work in an innovative and solutions focused way, including reflecting and learning as you go, and a willingness to flex and adapt approaches as needed
- ORGANISATION – Strong time management and the ability to manage to prioritise tasks
- COMPUTER LITERACY – You will be IT literate and have experience of working with multiple software programs such as Microsoft Office and Outlook.
What is a nice to have
- NVQ/L2 in Health and Social Care or equivalent
- Experience of working within a psychologically informed working environment
- Knowledge of housing management and IT databases
- Awareness of benefits and how they work including Universal Credit and other welfare systems
- Experience of finding positive moves on into permanent accommodation
